KeystoneCare is seeking a compassionate and dedicated Hospice Face-to-Face Nurse Practitioner to join our team. In this pivotal role, the Nurse Practitioner will conduct face-to-face assessments for patients receiving hospice services in alignment with Medicare requirements. You will play a critical role in ensuring that patients receive the appropriate level of care and support during their end-of-life journey.
Your expertise will help bridge the gap between patients, families, and the healthcare team, providing a holistic approach to care that addresses physical, emotional, and spiritual needs. You will collaborate closely with other healthcare professionals and families to develop and implement patient-centered care plans that honor the values and preferences of each patient.

Primary Job Responsibilities:
- Conduct face-to-face assessments for hospice patients to determine eligibility for hospice care
- Evaluate patients' conditions and update care plans as necessary to ensure compliance with all regulatory requirements
- Collaborate with the hospice interdisciplinary team, including physicians, nurses, social workers, and spiritual care providers
- Communicate effectively with patients and their families to provide guidance and support in the hospice care process
- Document assessments and findings accurately in a timely manner to ensure continuity of care
- Educate patients and families about hospice services and provide resources to support their needs
Requirements
- Graduate of an accredited nurse practitioner program
- Current Nurse Practitioner license in good standing to practice in the Commonwealth of Pennsylvania
- Minimum of two years of clinical experience in hospice or palliative care settings preferred
- ACHPN certification preferred or the ability to obtain within six months of hire
- In-depth knowledge of hospice regulations and Medicare policy
- Ability to communicate compassionately and effectively with patients and families during challenging times
- Strong organizational skills and attention to detail
- Valid driver's license and reliable transportation required
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
