Full Job Description
We are looking for an experienced and dedicated advanced practice provider (APP) to join our safety-net clinic APPNA Community Health Center of the SF Bay Area providing free of cost quality health care services to the under-served community members. You will provide direct patient care, discuss patient symptoms and concerns, perform thorough examinations to diagnose their medical problem, and prescribe medication or create a treatment plan if necessary. This is a Sunday only job from 9am-1pm. When required, you will collaborate with our physicians to treat more advanced illnesses.
Our ideal candidate has a current state license to practice and at least one year of NP experience in an ambulatory setting.
Job Responsibilities:
Work with physician colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to: taking histories, performing physical examinations, coordinating with outside providers as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures.
Evaluate for and order referrals for consultations, physical therapy, order radiographic imaging or diagnostic tests, and specialized treatments.
Provide patient and family education, arrange follow-up appointments, and engage in any approved privileges.
To evaluate, assess and create a plan of treatment per patient’s presenting HPI or established plan of care and clearly document in EHR.
To independently evaluate routine, acute, critical, new patients, or consultations when needed or required and discuss with attending and staff as appropriate and document in the EHR. Any required documentation workflow must be adhered to.
Be able to interpret and act upon ordered tests, procedures, and radiographic studies.
To assist in formulating and implementing protocols to improve efficiency in the clinic.
Effectively communicate with family or patients in person, via phone or email in a timely manner. Ensure in their absence, appropriate clinical coverage has been arranged for patient management and communication as appropriate.
At all times adhere to all policies and procedures of APPNA Community Health Center in terms of HIPAA/Communication/Privacy/Code of Conduct, and any other Clinic Policies & Bylaws.
Function as appropriate to their “scope of practice.” In general, the “scope of practice” includes the management of chronic stable medical conditions, routine care, acute care, and medical procedures per their approved standardized procedures. Familiarity with state statute for their licenses is necessary.
The APP is accountable to, evaluated by, and provided feedback by the supervising/collaborating physician or physician designee(s), the Practice Manager, and other colleagues’ familiar with the clinical activities.
Uses effective communication skills with patients and staff; shows sensitivity to differences of culture; demonstrates a positive and supportive manner for all patients, families, and colleagues.
Exhibits teamwork skills to positively acknowledge and recognize other colleagues.
Demonstrates an understanding of and adheres to privacy, confidentiality, and security policies and procedures related to Protected Health Information (PHI) or other sensitive and personal information.
Demonstrates an understanding of and adheres to safety and infection control policies and procedures.
Assumes accountability for improving quality metrics associated with clinic targets.

Pay: $65.00 - $75.00 per hour

Requirements
- Proven experience as Nurse Practitioner in a clinic, hospital, ER or other setting
- Expertise in primary care, acute care and chronic care practices
- Knowledge of health and patient care regulations
- Excellent communication skills
- Strong ethics
- Team spirit with a positive attitude
- Patience and cool-headedness
- Willingness to continue gaining knowledge and clinical experience
- Valid nurse practitioner license
- Master’s degree; PhD is a plus
